- A prescription, order, or referral is needed from the child's pediatrician.
- We will call to schedule your evaluation and review intake information, once the referral is received. During this time you will have the opportunity to discuss your concerns and learn about the services we provide.
- You will receive an email with an invitation to complete intake paperwork in our patient portal, along with your appointment information.
- A copy of your insurance card will be required at the initial appointment.
